When individuals register for the 2007 WADA
Workshop, they also can sign up for a
complimentary WADA Membership Benefit Gift. The form that you need to use is
posted on the WADA web site -- in the WADA
online registration packet index.
To receive the Benefit Gift at the Workshop, you MUST return the appropriate
form
by FRIDAY, SEPT. 28. Note: When the final version was originally posted,
the “size MEDIUM” choice was inadvertently omitted. An updated Order Form
has been posted. Sorry for
the inconvenience.
If you want, you can order MORE than one item via the Benefit Form. The first item
is free, each additional item is $35.00 apiece. Or, you can designate that the WADA
contribute $35.00 toward the WADA Scholarship
fund. Complete details are
outlined
on the 2-page Benefit Form.
The form to register for the Workshop is IN
ADDITION to the Benefit Form.
You MUST send in BOTH forms… one form to register for the Workshop and
the other to receive a WADA membership benefit gift at the 2007 event.
