at the WADA Workshop in November in Appleton. You can sign up for
this credit each year that you attend the WADA Workshop.

Registration packet used to register for the Workshop.

Prior to the Workshop, you MUST download ALL of the four items

below. Each link is a pdf file – allow time for your computer to open
all of the pages in these files:

Download ALL four of the Viterbo items in ONE packet
Allow plenty of time for this 8-page packet to open.

Requirements:  Brief write up of each session from the WADA Workshop
and a 2-3 page paper telling how you will apply what you learned at the
Workshop to your job. The paper and write-ups are due approximately
2 weeks following the Workshop.

Registration & Payment: The Registration Form needs to be completed.
This form is IN ADDITION to the form needed to register for the Workshop.
Send that form, and payment, to the address in West Allis… as listed on the form.
